PATIENT POLICIES
VISITATION:
VISITING HOURS: WEDNESDAYS 3:00-4:30PM & SUNDAYS 2:15-4:30PM
Visitors and patients may meet in the dayroom, dining or alcove areas. Visitors purses and cell phones are not allowed in the facility during visiting hours. Items for the patient must be left with the receptionist. Visitors under the age of 12 years will be allowed on the unit to visit with adult supervision and with unit nurse approval.
ONLY INDIVIDUALS WHO KNOW THE PATIENT CODE NUMBER WILL BE ALLOWED TO VISIT. THE CODE NUMBER IS ASSIGNED TO EACH PATIENT ON ADMISSION, AND IS NECESSARY FOR CONFIDENTIALITY.
CONFIDENTIALITY:
Visitors and patients are asked to respect the privacy and confidentiality of all individuals coming into the facility for care. When visitors call or want to see you, and they do not have a code number, we will protect your privacy and say, “we have no information”.
If for any reason, there is a need to share information with someone not employed here (for example, a therapist), you will be asked to sign a information release form.
PERSONAL EFFECTS: Upon admission, all valuables are requested to be given to family. Need to bring:
3 changes of casual clothing (No strings or belts)
Toothpaste/ toothbrush
Comfortable Walking Shoes (No strings)
Brush/Comb
Robe, Pajamas & Slippers
1 Sweater/Jacket
Deodorant (non-aerosol)
Shampoo/Conditioner
Contact lens care kit
SMOKING:
GULF OAKS is a smoke free facility. Smoking may be allowed outside on the patio at specified times with the doctors permission only.
